Job summary

Evolve-Therapy-Services seeks a Physical Therapist to provide in-house skilled nursing facility services and lead therapy programs for residents.

The role requires a BS in Physical Therapy and an active state license, with ongoing professional development and CMS documentation in NetHealth EMR. You will collaborate with an interdisciplinary team to plan and implement effective therapies.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.
• CMS documentation expectations in NetHealth EMR systems.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Document and bill physical therapy services in accordance with procedures.
• Evaluate residents to gather data for treatment planning and follow-up.
• Develop treatment plans with short and long-term goals, and adjust as needed.
• Communicate findings and recommendations to residents, families, and interdisciplinary team.
• Conduct regular screenings to determine need for intervention and treatment.
• Perform all duties to ensure high-quality resident care and compliance.
