Job summary

Evolve Therapy Services, LLC in Omaha, NE is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility. This PRN position focuses on delivering high-quality PT services and adherence to CMS documentation requirements within NetHealth EMR.

You will evaluate residents, plan personalized treatments, and collaborate with families and an interdisciplinary team to ensure optimal resident outcomes.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents following established standards of practice and department procedures.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Screen residents regularly to determine need for intervention/treatment.
• Screen referrals promptly to determine need for intervention.
• Evaluate residents to obtain data for treatment planning and implementation.
• Conduct specialized evaluations as indicated.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and the interdisciplinary team.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.
• Perform additional duties as required to discharge responsibilities.
