Job summary

Evolve Therapy Services in Oklahoma City, OK is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility. This PRN role focuses on delivering high-quality therapy services to residents, performing regular screenings, and developing individualized treatment plans within CMS guidelines.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to CMS documentation standards and professional development.

Responsibilities

• Provide physical therapy services to residents in accordance with standards.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound judgment in evaluation, planning, and follow-up of therapy programs.
• Conduct regular screenings to determine need for intervention.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Communicate evaluation findings and plans to residents, families, and the inter-disciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
