Job summary

Evolve-Therapy-Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility. You will provide comprehensive PT services, evaluate residents, develop treatment plans, and collaborate with the interdisciplinary team to ensure quality resident care.

Requirements include a Bachelor of Science in Physical Therapy, an active state license, ongoing continuing education, and documentation in NetHealth.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and facility procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention or treatment.
• Develop treatment plans with short and long-term goals and methods to achieve identified goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and interdisciplinary team.
