Job summary

Evolve Therapy Services is seeking a licensed Physical Therapist for an in-house Skilled Nursing Facility in Tavernier, FL. Roles include full-time, part-time, or PRN, with duties to plan, direct, and provide PT services ensuring quality resident care.

Responsibilities include evaluation, goal setting, documentation in NetHealth EMR, and collaboration with an interdisciplinary team. Valid Florida PT license and ongoing CE are required.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Ability to evaluate residents and develop treatment plans.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Document and bill for PT services in accordance with CMS expectations.
• Conduct screenings and evaluations to determine intervention needs.
• Develop and implement short- and long-term treatment goals.
• Communicate findings and recommendations to residents, families, and the care team.
