Qualifications

• Active Physical Therapy license in state(s) of practice.
• Bachelor of Science in Physical Therapy from an accredited program.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents according to established standards and departmental procedures.
• Document and bill physical therapy services in accordance with procedures.
• Evaluate residents to plan and implement therapy programs and goals.
• Develop treatment plans with short and long-term goals and communicate findings to residents and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
• Perform additional duties as required by the organization.
