Job summary

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with standards and procedures.
• Document and bill therapy services in CMS-compliant EMR systems.
• Develop and implement short and long-term treatment plans with measurable goals.
• Screen residents regularly to determine need for intervention/treatment.
• Evaluate residents to obtain data for planning and implementation.
