Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Key West, FL. The role supports residents with comprehensive PT services and requires active licensure in the state.

The position offers full-time, part-time, or PRN options with competitive pay and benefits. Responsibilities include evaluating, planning, and documenting therapy plans in the CMS-compliant NetHealth EMR, while collaborating with an interdisciplinary team.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of therapy programs.
• Screen residents regularly to determine need for intervention.
• Screen residents referred to physical therapy to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Interpret and communicate evaluation findings and plans to residents and families.
• Complete CMS-compliant documentation in NetHealth EMR.
