Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Tulsa, OK. The position is PRN and focuses on delivering high-quality therapy to residents.

The PT will plan, direct, and document therapy services, collaborate with the interdisciplinary team, and ensure compliance with CMS and licensing requirements while promoting resident independence and quality of life.

Qualifications

• Must have a Bachelor of Science in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards, procedures and productivity targets.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate residents to gather data for treatment planning and implementation; develop and follow treatment plans.
• Conduct screenings to identify intervention needs and determine appropriate treatment.
• Communicate evaluation findings, treatment plans and recommendations to residents, families and interdisciplinary team.
• Perform CMS-compliant documentation in NetHealth EMR and complete required records.
• Collaborate with the interdisciplinary team to discharge responsibilities effectively and responsibly.
