Evolve Therapy Services is seeking a compassionate Physical Therapist to join our in-house skilled nursing facility team in Ponca City, OK. You will evaluate residents, develop treatment plans, and deliver therapy in accordance with CMS guidelines and facility procedures.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Conduct evaluations to develop treatment plans with short and long-term goals.
• Develop treatment plans and communicate findings to residents and interdisciplinary team.
