Job summary

Evolve Therapy Services in Ponca City, OK seeks an Occupational Therapist for an in-house Skilled Nursing Facility. Part Time or PRN, you will plan, organize, and direct occupational therapy services to ensure high-quality resident care in compliance with applicable standards.

Responsibilities include screening, evaluations, treatment planning, scheduling, therapy delivery, and coordination with the interdisciplinary team. Licensure in the practicing state and ongoing CE are required.

Qualifications

• Need a Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to identify best interventions per patient.
• Evaluate residents to inform and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings, plans, and recommendations to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy per treatment plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments meet reimbursement requirements.
