Evolve Therapy Services in Muskego, Wisconsin is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility. The PTA will work under the supervision of a Physical Therapist to provide therapy services, conduct screenings, and help implement treatment plans for residents.

Required qualifications include an Associate Degree in Physical Therapy and an active PTA license. This PRN role emphasizes teamwork, resident education, and accurate documentation in NetHealth

Responsabilidades

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluations and in treatment plan development with the Physical Therapist.
• Help establish and modify resident schedules for treatment with the PT and team.
• Train residents, families, and staff in equipment use.
• Document in NetHealth EMR per CMS expectations.
• Contribute to quality assurance activities and resident education.
