Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Le Center, MN. This full-time, part-time, or PRN role focuses on delivering occupational therapy services within CMS guidelines and collaborating with an interdisciplinary team.

Responsibilities include screening, developing treatment plans, scheduling therapies, documenting care, and ensuring billing accuracy.

Qualifications

• Associate's degree in Occupational Therapy is required.
• MUST have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Screen residents to determine best intervention per scope of practice.
• Implement treatment plans and follow short- and long-term goals under supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules with team members.
• Conduct therapy according to treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.
