Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to support an in-house Skilled Nursing Facility in Rochester, MN. This full-time role involves planning, organizing, and directing OT services for residents under the supervision of a licensed therapist.

The candidate must have an Associate's degree in Occupational Therapy and active OTA licensure in the state(s) of practice, plus commitment to ongoing professional development and compliance with CMS standards.

Qualificações

• Associate's degree in Occupational Therapy is required.
• Active OTA licensure in state(s) of practice is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsabilidades

• Conduct regular screening of residents to determine best intervention or treatment based on scope of practice.
• Perform treatment plans for residents; follow short- and long-term goals set by the supervising therapist.
• Interpret and communicate evaluation findings, treatment plans and recommendations to residents, families, and the interdisciplinary team.
