Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to support an in-house Skilled Nursing Facility in Le Center, MN. The OTA will plan, organize, develop and direct Occupational Therapy Services to ensure the highest quality resident care in accordance with federal, state, and local standards.

Qualifications

• Must have active OTA licensure in state(s) of practice.
• Associate's degree in Occupational Therapy.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment.
• Perform treatment plan to residents; follow goals based on plan of care.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and team members.
• Establish individual resident therapy schedules in cooperation with team members.
• Conduct therapy according to plan and ensure proper documentation and billing.
• Comply with CMS documentation standards daily.
