Job summary

Evolve Therapy Services, LLC is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility located in Minneapolis, MN. This role supports occupational therapy services, develops treatment plans, and ensures quality resident care under supervision of a licensed therapist.

The position offers Full Time, Part Time, or PRN schedules with opportunities for growth. Must hold an active OTA license in the applicable state and have an Associate's degree in Occupational

Qualifications

Associate's degree in Occupational Therapy required.
Must have active OTA licensure in state(s) of practice.
Ongoing continuing education and professional development to maintain licensure.

Responsibilities

Screen residents to determine best intervention per plan of care.
Perform treatment according to established plan and goals.
Communicate findings and recommendations to residents, families, and team.
Create individual therapy schedules with team collaboration.
Deliver therapy and ensure proper documentation and billing.
