Job summary

Evolve Therapy Services in West Saint Paul, MN is seeking a Certified Occupational Therapy Assistant for our in-house Skilled Nursing Facility.

In this role you will plan, organize, develop, and direct OT services in coordination with the supervising therapist, ensuring high-quality resident care.

The position offers both full-time and PRN options, with opportunities for growth and ongoing professional development.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine interventions.
• Perform treatment plans under supervision to meet goals.
• Interpret and communicate evaluation findings to residents and families.
• Establish individual resident therapy schedules with team members.
• Deliver therapy per plan and ensure proper documentation and billing.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily.
