Job summary

Everstand in West Virginia seeks a Wraparound Facilitator to engage high-need youth and families in developing, implementing, and monitoring an intensive, individualized care plan within a care team. This role requires coordinating intake, meetings, and resources to support a strengths-based plan.

You will maintain documentation, ensure fidelity to the wraparound model, and provide 24/7 on-call support as part of a collaborative, community-based team.

Qualifications

• Demonstrated communication and interpersonal skills.
• Experience collaborating with multi-disciplinary teams.
• Ability to formulate and implement care plans with fidelity to models.

Responsibilities

• Model Everstand values in interactions with youth and families.
• Coordinate intake and assessments to identify needs and strengths.
• Develop and monitor individualized wraparound care plans with measurable outcomes.
• Facilitate family meetings and ensure timely documentation and reports.
• Provide on-call support and crisis response as needed.
