Job summary

Care Connectors is seeking Nurse Practitioners to perform in-home Annual Wellness Visits for Medicare members. Visits are conversational, education-focused and conducted in the patients' homes across a local region.

The role offers flexibility with part-time or full-time options, paid training, liability insurance, and pre-scheduled patient appointments. Travel within a 10 mile radius is required; bilingual Spanish is a plus.

Qualifications

• Active NP license and board certification in California.
• Prefer at least 3 months of wellness exam experience.
• Ability to travel within a region (visits clustered in a 10 mile radius).
• Spanish speaking a plus.

Responsibilities

• Conduct in-home wellness assessments.
• Educate members on preventive care benefits.
• Identify gaps in care to reduce avoidable ER visits.
• Complete documentation using EMR tools.
• Visitations occur in members' homes for personalized care.

Requirements

• Active State NP license and Board Certification

• Comfort with EMR/documentation systems
• Ability to travel within a region (visits are clustered in a 10 mile radius)
