Job summary

The Salvation Army in Omaha, NE seeks a full-time VA Support Services Counselor to provide in-home rehabilitation for Veterans, aiming to maintain community living and reduce higher-level care needs.

Responsibilities include assessments, ISP development within 7 days, ongoing risk and needs evaluations, and regular contact with participants. Requires an Associate’s degree in psychology/social work and at least 1 year of direct care experience.

Qualifications

• Associates Degree required in psychology, social work, or related field; B.S. preferred.
• Bachelor’s Degree preferred.
• At least one year of direct care experience with individuals with severe mental illness or similar.

Responsibilities

• Complete ISP within 7 days of admission.
• Maintain ISP and risk assessments every 90/180 days.
• Minimum of three face-to-face contacts per month.
• Provide support in daily living skills, budgeting, and medication adherence.
• Develop strategies to engage individuals in mental health treatment.
• Coordinate with VA liaison for treatment and rehabilitation.
• Maintain case records and documentation.
• Participate in treatment team discussions on progress and barriers.
