Job summary

The Salvation Army is seeking a full-time in-home rehabilitative services worker to support Veterans in maintaining community living. You will develop and monitor Individual Service Plans and collaborate with the VA liaison, delivering essential life skills and relapse prevention strategies.

Qualifications include an Associate's Degree (Bachelor's preferred) and at least one year of direct care experience; must pass background checks and have a valid driver's license.

Qualifications

• Associates Degree required; Bachelor's preferred; minimum one year direct care experience.
• Ability to be flexible and manage multiple projects.
• Experience with Microsoft Office and data entry.
• Strong communication and confidentiality.
• Must pass background checks and have a valid driver's license.

Responsibilities

• Conduct comprehensive assessments and ISP development within 7 days.
• Maintain ISP and conduct regular risk assessments and GAIN screening.
• Provide daily living skills support and relapse prevention.
• Coordinate with Veterans Administration Liaison and document records.
• Prepare crisis/relapse plans and respond to emergencies.
• Drive as part of job responsibilities.
• All other duties as assigned.
