Job summary

The Salvation Army is seeking a full-time in-home rehabilitative services worker to support veterans and their families in maintaining community living. The role involves completing assessments, developing service plans, and coordinating with VA and treatment teams.

Education requires an associate degree with preferred bachelor's; experience in mental health services is expected. Strong communication and organizational skills, plus driving, are essential.

Qualifications

• Associates Degree required from a two-year college or university in psychology, social work, or related field.
• Bachelor’s Degree preferred; experience in mental health services.
• At least one year of direct care experience with participants with severe mental illness or other mental health services.
• Ability to be flexible and able to work on multiple projects or tasks simultaneously.
• Proficient in Microsoft Office and able to access timekeeping via a web-based system.

Responsibilities

• Complete comprehensive assessment and develop ISP within 7 days of admission.
• Maintain supervision of ISP and ongoing assessments, risk assessments, and update ISP every 90/180 days.
• Must have a minimum of three face-to-face contacts per month.
• Provide support interventions focusing on daily living skills, budgeting, medication compliance, and relapse prevention.
• Develop and implement strategies to engage the individual in mental health treatment services.
• Perform case management activities with Veterans Administration Liaison.
• Maintain case records, statistics and assist with reports and manuals.
• Complete required case file documentation (ISPs, notes, intake/discharge paperwork).
• Participate with treatment team on progress and barriers; address clinical issues.
• Prepare crisis/relapse plans and respond to crisis to transition back to community.
