Job summary

Vinfen is seeking an In-Home Support Staff member for the Agency with Choice program to deliver direct case management and life-skills training. The role emphasizes advocacy, collaboration with families, and helping residents engage with their communities.

The successful candidate will participate in supervision, training, and data collection, while supporting daily living activities, transportation needs, and safety compliance.

Qualifications

• Three months to one year experience in a related function.
• Driving is required with a valid license and safe driving record.
• Able to lift at least 25 pounds and perform physical tasks as needed.

Responsibilities

• Provide training to persons served as determined by family/guardian and person served.
• Assist persons served in becoming fully integrated into their communities.
• Monitor residents’ physical and emotional needs and report changes.
• Escort and provide transportation for residents as needed.
• Maintain records and collect data to track ISP goals.
• Implement service plan interventions and provide direct training.
• Act as an advocate for persons served and report incidents as required.
• Perform housecleaning tasks and provide coverage for the program as agreed.
