We are seeking a Direct Support role with a pay rate of $16.68/hour. Schedule includes evenings and weekends. Valid driver's license and clean MVR are required.

Qualifications

• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service plans, including socialization, health maintenance, and skill development.
• Assist with daily activities such as personal care, housekeeping, transportation, and community integration.
• Accurately document progress and maintain records confidentiality.
• Follow safety policies and report incidents per Sevita guidelines.
