Job summary

REM Community Services, a part of Sevita, is hiring Direct Support Professionals to provide direct support to individuals with intellectual and developmental disabilities in residential settings and periodic programs. This role requires a valid driver’s license, a high school diploma or equivalent, and 6 months of experience in human services.

You will document progress, assist with daily activities, and help individuals achieve health and personal goals.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Accurately document progress and activity; reviews records and logs to stay abreast of changes in service plans; maintains confidentiality.
• Maintain healthy and professional relationships with individuals, families, guardians and case managers; greets family members and other visitors; implements Sevita Customer Service Standards.
• Reports any instance of alleged abuse or neglect; reports medical, behavioral and other incidents in accordance with Sevita policy and external requirements.
