REM Community Services is seeking Direct Support Professionals in non-exempt, full-time or part-time, hourly roles. You will deliver direct support within a single program, typically assisting 4–6 individuals in residential settings or up to 10 in periodic

Qualifications

High school diploma or equivalent preferred.
Six months of experience in human services preferred.
Must be 18 years of age.
Working knowledge of computers.

Responsibilities

Deliver support per individual service plans, including socialization, health maintenance, medication administration, and skill/behavioral development.
Assist with daily activities: personal care, housekeeping, recreation, transportation, community integration, shopping, and other activities of daily living.
Accurately document progress and activity; maintain confidentiality.
Maintain healthy, professional relationships with individuals, families, guardians and case managers.
Report abuse, medical, behavioral incidents per policy and external requirements.
Assist with health care needs; monitor health and relay information to nurses as required.
