Job summary

CenterWell Home Health is seeking a Speech-Language Pathologist to deliver therapy in patients’ homes or facilities, reporting to the Clinical Manager. You will develop treatment plans, address speech, language, voice and swallowing disorders, and collaborate with families and care teams to improve communication and quality of life.

Required ASHA Certification, at least six months of experience, a valid license and CPR. A valid driver’s license and reliable transportation are required.

Qualifications

• ASHA CCC required or Certification
• Minimum six months experience as a speech-language pathologist
• Current unrestricted license and CPR certification

Responsibilities

• Evaluate, direct and provide speech-language pathology services in patients' homes or facilities
• Develop and periodically review the Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, language, voice, hearing and swallowing and administer diagnostic tests
• Observe, record and report changes to supervisor or physician and maintain documentation
• Provide instruction to patients and families; coordinate care and discharge planning
