Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home or facility-based care, reporting to the Clinical Manager. You will evaluate, diagnose and treat speech, language, voice and swallowing disorders, develop treatment plans, and coordinate care with the team to help patients regain independence and improve quality of life.

The role requires ASHA CCC or equivalent, at least six months of experience, a valid license and CPR certification.

Qualifications

• Requires ASHA CCC or equivalent
• Minimum six months experience as a speech-language pathologist
• Current and unrestricted license
• CPR certification required
• Valid driver’s license with vehicle insurance and reliable transportation

Responsibilities

• Evaluate and provide speech-language pathology services in home or facility
• Develop and review Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing and swallowing
• Administer and interpret diagnostic tests and therapeutic applications
• Observe and report changes in patient condition to supervisor/physician
• Instruct patients in alternative communication methods when appropriate
• Counsel patients and families; maintain documentation per policy
• Participate in care coordination and discharge planning
• Maintain confidentiality and comply with privacy standards
