Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home therapy, helping patients regain speech, swallowing, and communication skills. You will assess and treat needs directly in patients’ homes or facilities, and collaborate with care teams to customize plans of care for each individual.

The role requires documentation, ongoing evaluation, Plan of Treatment updates, and adherence to HIPAA and safety protocols in home health settings.

Qualifications

Six months minimum experience as a Speech-Language Pathologist.
Current and unrestricted license.
Current CPR certification.
Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

Evaluate and provide speech-language pathology services in the home or facility.
Develop and review the Plan of Treatment and Plan of Care.
Assess and treat disorders of speech, voice, language, hearing and swallowing.
Administer and interpret diagnostic tests and therapeutic applications.
Observe, record and report changes in patient condition to supervisor/physician.
Provide instruction on alternative communication systems when appropriate.
Counsel patients, families and healthcare staff.
Maintain and submit documentation and care plans as required.
Participate in care coordination and discharge planning.
