Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in home or facility settings. You will develop and review Plans of Treatment and Care, assess speech, language, voice and swallowing disorders, and coordinate care with the healthcare team to improve patient independence.

The role requires six months of SLP experience, a current ASHA CCC-SLP/CCC-A, licensure, CPR, and a valid driver’s license.

Qualifications

• Must hold Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA).
• Minimum of six months of experience as a speech-language pathologist.
• Current and unrestricted professional license.
• CPR certification current.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Evaluate, direct and provide speech-language pathology services in home or facility settings.
• Participate in development and review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing; administer and interpret diagnostic tests.
• Observe, record and report patient condition changes; communicate with supervisor/physician.
