CenterWell Home Health is seeking a licensed Speech-Language Pathologist to deliver in-home or facility-based therapy, evaluate and treat communication and swallowing disorders, and participate in care planning. The role emphasizes patient education, documentation, and coordination with the care team.

Minimum six months of home health or clinical SLP experience, current ASHA certification, and a valid driver’s license with reliable transportation are required. CPR certification is also expected.

Qualifikationen

• Certification in Speech-Language Pathology (ASHA) is required.
• Minimum six months of experience as a speech therapist/speech-language pathologist.
• Current and unrestricted license to practice.

Aufgaben

• Evaluate, direct and provide speech-language pathology services to patients in the home or facility.
• Participate in development and review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to minimize impact.
• Administer and interpret diagnostic tests and therapeutic applications.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide instruction in alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff; maintain documentation and progress reports.
• Participate in care coordination and discharge planning.
