Job summary

CenterWell Home Health seeks a licensed Speech-Language Pathologist to deliver in-home or facility-based speech-language pathology services. You will evaluate patients, develop treatment plans, and work with the care team to promote rehabilitation and independence.

Requirements include ASHA CCC-SLP/CCC-A, at least six months of experience, a valid license and CPR certification, plus a valid driver’s license with transportation. This role involves travel to patient homes within the service area.

Qualifications

• ASHA CCC-SLP or CCC-A certification required.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license to practice.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation required.

Responsibilities

• Evaluate and provide speech-language pathology services in patients’ home or facility.
• Develop and periodically review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic treatments.
• Maintain documentation and submit reports as required.
• Participate in care coordination and discharge planning.
