Job summary

CenterWell Home Health is seeking a Speech Therapist to assess, diagnose and treat speech, language, cognitive, voice and swallowing disorders in patients’ homes or facilities. You will document progress and coordinate care with the team.

Responsibilities include developing plans of treatment, guiding patients and families, and ensuring compliance with professional standards and company policy. A valid driver’s license and CPR are required.

Qualifications

ASHA CCC-SLP certification or equivalent.
Minimum 1 year experience as Speech-Language Pathologist.
Current and unrestricted license to practice.
Current CPR certification.
Valid driver’s license, auto insurance and reliable transportation required.

Responsibilities

Evaluate, direct and provide speech-language pathology service to patients in home or facility.
Participate in development/review of Plan of Treatment and Plan of Care.
Assess and treat disorders of speech, voice, language, hearing and swallowing.
Administer and interpret diagnostic tests and therapeutic applications.
Observe and report changes to supervisor/physician.
Provide instruction on alternative communication systems when appropriate.
Counsel patients, families and healthcare staff.
Prepare and submit written evaluations, visits, plans and progress reports.
