Job summary

CenterWell Home Health is seeking a Home Health Speech-Language Pathologist to evaluate, direct and provide SLP services in patients' homes or facilities. You will participate in treatment planning, assess and treat communication and swallowing disorders, and document progress for care coordination and discharge planning.

The role requires ASHA CCC/licensure, at least six months of experience, current CPR, a valid driver's license and reliable transportation. Home health experience is preferred.

Qualifications

• Meets the education and experience requirements for ASHA Certification of Clinical Competence in Speech-Language Pathology or Audiology
• Minimum six months experience as a speech-language pathologist
• Current and unrestricted license
• Current CPR certification
• A valid driver’s license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize effects
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audiologic screening
• Observe, record and report changes in the patient’s condition and response to treatment to supervisor and/or physician
• Provide instruction and training to patients in use of alternative communication systems when appropriate
• Provide counsel and instruction to patients, families and healthcare staff
• Maintain and submit documentation as required by the Company and/or facility; prepare reports as required
• Participate in care coordination activities and discharge planning
• Maintain highest standards of professional conduct concerning confidentiality of patient information
