Job summary

RallyX is seeking a licensed Speech-Language Pathologist to deliver in-home services in Florida. Join a care-driven team providing skilled medical home care and clinical nursing support.

The role includes assessments, treatment planning with patients and physicians, documentation, and collaboration with a multidisciplinary staff. Requires Florida licensure, Master’s/Doctoral in SLP, and CPR certification.

Qualifications

• Must have a Master’s or Doctoral degree in speech-language pathology.
• Licensed to practice within the state of Florida.
• CPR certification required (Online certification not accepted).
• Member of the American Speech-Language-Hearing Association preferred.
• At least one year of institutional experience.

Responsibilities

• Provide Speech Pathology services and assessments to patients in their homes.
• Evaluate patients’ level of functioning and recommended mechanisms to enhance patient communication.
• Establish a Speech Therapy treatment plan with patient and primary physician.
• Assist in the implementation and development of patient care plans.
• Prepare progress notes following visits for patients’ charts.
