Job summary

Palms Home Care is seeking a dedicated Speech Language Pathologist to provide in-home therapy services in Orlando, FL. The role focuses on evaluating, treating, and coordinating care for patients with speech, language, cognitive, and dysphagia disorders.

The successful candidate will have a Florida SLP license, a Master’s degree in Speech Language Pathology, and experience in home health. CPR certification and a valid driver’s license are required.

Qualifications

• In-home therapy focus requiring strong evaluation and treatment planning.
• Knowledge of Medicare/OASIS guidelines is a plus.
• CPR certification and clean driving record required.

Responsibilities

• Perform comprehensive evaluations and OASIS assessments
• Develop and implement individualized care plans
• Treat patients with speech, language, cognitive, and dysphagia disorders
• Document visits per Medicare guidelines
• Educate patients and caregivers on exercises and compensatory strategies
• Coordinate effectively with physicians and care team members
