Job summary

CenterWell Home Health is seeking a skilled Speech-Language Pathologist to deliver in-home therapy focused on rehabilitation and functional improvement. You will evaluate and treat patients, participate in care plans, and coordinate with the health team to support independence and quality of life.

The role requires ASHA CCC-SLP, state licensure, CPR, and reliable transportation for home visits. A strong emphasis on patient-centered care and documentation is essential.

Qualifications

• Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA)
• Minimum six months experience as a speech therapist / speech language pathologist
• Current and unrestricted professional license
• Current CPR certification
• Valid driver’s license, auto insurance and reliable transportation

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize effects
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audiology screening
• Observe, record and report changes in patient condition to supervisor/physician
• Provide instruction and training to patients in use of alternative communication systems when appropriate
• Provide counsel and instruction to patients, families and healthcare staff
• Maintain and submit documentation as required; prepare written reports and care plans
• Participate in care coordination and discharge planning
• Maintain confidentiality of patient information
