Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home SLP services and contribute to plans of care. You will assess, treat, and monitor patients'' speech, language, voice and swallowing disorders, aimed at improving function and independence.

You will work with a multidisciplinary team, document thoroughly, and engage in care coordination and discharge planning. A valid state license and ASHA certification are required.

Qualifications

• ASHA CCC-SLP or CCC-Aud certification required.
• Six months minimum experience as a speech-language pathologist.
• Current, unrestricted professional license required.
• Current CPR certification required.
• Valid driver''s license, auto insurance and reliable transportation required.

Responsibilities

• Evaluate, direct and provide speech-language pathology services in patient homes or facilities.
• Contribute to Plan of Treatment and Plan of Care development and review.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapies, including audio-logic screening.
• Observe and report changes in patient condition and response to treatment to supervisor/physician.
• Provide training to patients in use of alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff; maintain documentation per policy.
• Prepare reports of evaluations, visits, care plans and progress as required.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and professional conduct in all interactions.
