Job summary

Creative Solutions Services, LLC is seeking a Speech Language Pathologist (SLP) for an onsite 17-week assignment in Chambersburg, PA. The position provides outpatient rehabilitation services primarily in a home-based setting, with a flexible schedule across various environments.

The SLP will assess and treat older adults, developing and implementing therapy plans, and coordinating with an interdisciplinary team to optimize communication and swallowing abilities.

Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice.
• Must possess or be eligible for a Certificate of Clinical Competence from ASHA (CCC) or be eligible for ASHA Clinical Fellow.
• Must have a valid driver’s license or reliable transportation.

Responsibilities

• Supervise CFYs, students, and support personnel in accordance with licensure requirements.
• Adhere to department and patient treatment schedules and complete clinical and billing documentation.
• Attend patient care conferences and other related meetings.
• Collaborate with rehabilitation team and physician to select appropriate therapy regimen.
• Develop and adjust plan of care according to patient needs.
• Coordinate home and community care with interdisciplinary team.
• Order supplies and equipment as necessary.
