Job summary

Creative Solutions Services, LLC seeks a Speech Language Pathologist to provide outpatient rehabilitation in home and community settings in Great Barrington, MA. The 16-week onsite contract runs 8 hours/day, 5 days/week, with a 40-hour minimum guarantee.

Requirements include a valid SLP license and ASHA CCC eligibility; driver’s license or reliable transportation is needed to service patients’ residences.

Qualifications

• Must hold a valid SLP license in the practicing state.
• Must have or be eligible for ASHA CCC or participate as ASHA Clinical Fellow.
• Must have a valid driver’s license or reliable transportation.

Responsibilities

• Provide outpatient rehab services in home or community settings.
• Coordinate with rehabilitation team to select appropriate therapy regimens.
• Document clinical notes and billing in a timely manner.
• Develop and adjust plans of care with patient goals and caregiver input.
• Ensure patient safety and adherence to HIPAA and ethics.
