Southwest Behavioral & Health Services is seeking a SMI Case Manager for the Buckeye Outpatient Clinic. The role includes direct in-home and community-based services, crisis intervention, and coordinating care with a multidisciplinary team to develop and revise service plans for adults with Serious Mental Illness.

The position requires a Bachelor’s degree and at least one year of behavioral health experience, with an option for a five-year combined education/experience path.

Qualifications

• Bachelor’s degree with one year of full-time behavioral health experience.
• Equivalent combination of education and experience totaling five years is acceptable.

Responsabilités

• Provide comprehensive in-home and community-based intervention to adults with Serious Mental Illness (SMI).
• Assess members per policies and clinical protocols; develop service plans based on assessments.
• Coordinate member care with the care team to review interventions and revise plans.
• Maintain relationships with members and support networks; provide guidance on treatment planning and resources.
• Monitor mental status and update treatment plans; deliver crisis intervention as needed.
