Southwest Behavioral & Health Services seeks a Case Manager, Medication Only for Buckeye Outpatient. You will deliver direct services in home, community, or office settings, including crisis stabilization.

The role emphasizes coordinating care and helping clients access community resources to support functioning and family/community integration. The position requires a Bachelor's degree with at least one year in behavioral health, plus First Aid/CPR certification.

Qualifications

• Bachelor’s degree with one year of full-time behavioral health experience.
• Certification in First Aid & CPR is required or obtainable.
• Fingerprint clearance issued by the Arizona Department of Public Safety or eligibility to obtain within 8 weeks.

Responsibilities

• Provide comprehensive support through in-home and/or community-based intervention to adults and their family members.
• Assesses consumers in accordance with policies, procedures and clinical protocols.
• Develops service plans based on assessment and diagnostic process.
• Coordinates consumer care with the care team and revises plans as needed.
• Monitors mental status and response to treatment plans through face-to-face contact.
• Provides crisis intervention in-person or by phone.
