Job summary

CenterWell Home Health seeks a Speech Language Pathologist to deliver in-home evaluations and therapy, reporting to the Clinical Manager. You will assess and treat disorders of communication and swallowing, and implement treatment plans to help patients regain independence.

The role requires ASHA CCC or eligibility, at least six months of experience, an active license, CPR certification, and a valid driver's license with insurance. In-home visits with comprehensive benefits are provided.

Qualifications

• Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA).
• Minimum six months experience as a speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology services to patients in home or facility.
• Participate in development and review of Plan of Treatment and Plan of Care.
• Assess and treat speech, voice, language, hearing and swallowing disorders.
• Administer and interpret diagnostic tests and apply therapeutic treatments.
• Observe, record and report changes to supervisor or physician.
• Provide instruction and training in alternative communication systems.
• Provide counsel to patients, families and healthcare staff.
• Maintain and submit required documentation.
• Prepare and submit reports of evaluations, visits, care plans and progress.
• Participate in care coordination and discharge planning.
• Maintain confidentiality of patient information.
