Job summary

Home Instead is seeking a compassionate Client Care Coordinator in Erie, PA for an in-office daytime role. The position offers 25–40 hours per week, typically Monday through Friday with minimal evenings, weekends, or holidays, and occasional local travel to client homes and community locations.

Qualifications

• Experience in healthcare, senior care, or client services preferred.
• Experience working with older adults and families is a plus.
• College degree preferred, but not required.

Responsibilities

• Field new client inquiries by phone with compassion and knowledge.
• Conduct service inquiries and care consultations with prospective clients and families.
• Develop customized care plans based on client needs.
• Maintain ongoing communication and follow-up with clients and families.
• Coordinate client care with office staff, schedulers, and Care Pros.
• Conduct client/Care Pro introductions for new assignments.
• Maintain accurate client and responsible party records.
• Address client and family concerns with empathy and urgency.
