Job summary

Home Instead Erie, PA is seeking a compassionate Client Care Coordinator to serve as advocate and primary contact for clients and their families, blending relationship-building, care coordination, and problem-solving to ensure quality service.

This is primarily an in-office daytime role (25–40 hours per week), with occasional local travel to client homes and community events. Benefits include medical coverage, PTO, 401(k), and a company vehicle.

Qualifications

• Healthcare or senior-care experience preferred.
• Experience working with older adults and families is a plus.
• College degree preferred but not required.

Responsibilities

• Field new client inquiries by phone with empathy.
• Develop customized care plans based on client needs.
• Maintain ongoing communication with clients and families.
• Coordinate client care with staff and Care Pros.
• Maintain accurate client records and documentation.
