2nd Home Skilled Care Services LLC is seeking a Registered Nurse (RN) for in-home visits in the Aurora, CO area. The role involves performing patient assessments, developing care plans, and documenting care while coordinating with physicians and caregivers.

Requirements include an active CO RN license and CPR/BLS certification. Prior home health or geriatric experience is preferred, with an emphasis on safe, patient-centered care in home settings.

Formación

• Current, unrestricted Registered Nurse license in the State of Colorado.
• Current CPR/BLS certification.
• Previous experience in adult day services, home health, long-term care, geriatrics, rehabilitation, community health, or a similar healthcare setting preferred.
• Experience working with older adults and individuals with disabilities preferred.
• Knowledge of medication management, nursing delegation, infection control, emergency procedures, and clinical documentation.

• Conduct skilled nursing visits and patient assessments
• Develop and update individualized care plans as assigned
• Complete accurate and timely clinical documentation
• Communicate with physicians, caregivers, and the clinical team
