Job summary

Amazing Care Home Health Services in Aurora, CO is seeking motivated LPNs with home health experience to join our team. You will provide skilled nursing visits to clients in their homes and support education and care planning.

You will assess patient needs, develop care plans, document thoroughly, and coordinate with clinicians and families. A Colorado LPN license, 1-2 years of nursing experience, and strong communication are required; we offer weekly pay and comprehensive benefits.

Qualifications

Valid LPN license in the state of Colorado.
Minimum of 1-2 years of nursing experience, preferably in home health or a similar setting.
Excellent clinical assessment and critical thinking skills.
Proficiency in computer usage and documentation.
Strong communication and interpersonal skills.

Responsibilities

Provide skilled nursing care to patients in their homes.
Work independently to assess patient needs, develop care plans, and implement appropriate interventions.
Maintain accurate and thorough documentation of patient assessments, interventions, and outcomes.
Communicate effectively with patients, families, team members, and other healthcare professionals to ensure coordinated care.
Adhere to all regulatory requirements and company policies and procedures.
