Qualifications

• Valid Driver's License & clean Motor Vehicle Record (MVR) required.
• One year nursing experience in home health preferred.
• Participation in on-call duties may be required.

Responsibilities

• Provide skilled nursing procedures, medications, and treatments as prescribed by a physician.
• Coordinate care plans, admits patients, and collaborates with interdisciplinary teams.
• Educate patients and caregivers on self-care, medications, diet, and preventive health measures.
