Job summary

Amazing Care, Inc. is seeking motivated RNs for Home Health Skilled Nursing Visits in Downtown Denver and Northeastern Aurora. In this role you will visit patients in their homes, assess needs, develop care plans, and provide skilled nursing interventions to support recovery and well-being.

You will document care, coordinate with families and clinicians, and ensure compliance with regulations. Colorado RN license and 1–2 years of home health experience preferred.

Qualifications

• Valid RN license in Colorado.
• 1–2 years nursing experience, preferably in home health.
• Strong clinical assessment and critical thinking skills.
• Proficiency in computer usage and documentation.
• Excellent communication and interpersonal skills.
• Compassionate, patient-centered approach to care.

Responsibilities

• Provide skilled nursing care to patients in their homes.
• Work independently to assess patient needs, develop care plans, and implement appropriate interventions.
• Maintain accurate and thorough documentation of patient assessments, interventions, and outcomes.
• Communicate effectively with patients, families, team members, and other healthcare professionals to ensure coordinated care.
• Adhere to all regulatory requirements and company policies and procedures.
