Job summary

Qualifications

• Current and unrestricted RN license in the state of practice (CT).
• Current CPR certification.
• Valid driver's license and access to reliable transportation.

Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by the physician.
• Coordinate care with other health professionals to maintain continuity of patient care.
• Complete nursing assessments per program requirements and payer needs.
• Ensure patient eligibility and medical necessity for services defined by payer and policy.
• Develop and revise individualized care plans with other providers.
